The 10-question Edinburgh Postnatal Depression Scale (EPDS) is a valuable and efficient way of identifying patients at risk for perinatal depression. The EPDS is easy to administer and has proven to be an effective screening tool. Mothers who score above 13 are likely to be suffering from a depressive illness of varying severity.



What is a Continuity of Care Document (CCD)?

The CCD is a global standard for electronically exchanging a patient health summary. The CCD contains a core data set of the most relevant administrative, demographic, and clinical information about your or a family members healthcare. Covering one or more healthcare encounters, it provides a means for one physician, hospital, or other clinical setting to aggregate all of your pertinent data and electronically forward it to other healthcare providers, to support the continuity of care.
Its primary use case is to provide a snapshot in time for a specific patient. These "snapshots" of a patients health data that are useful and possibly lifesaving, if available at the time of a clinical encounter.
Because it is expressed in the standard data interchange language known as XML, a CCD can potentially be created, read and interpreted by any electronic medical record (EMR) or electronic health record (EHR) software application.
